<br />12.2 <br /> <br />12.3 <br /> <br />City shall continue to contribute to the Santa <br />Ana Police Officer Medical Insurance Trust Fund <br />an amount toward the payment of premiums for <br />each employee who is enrolled in the long-term <br />disability insurance plan provided by the <br />Association for employees covered by this <br />Agreement in accordance with the following <br />schedule: <br /> <br />1. <br /> <br />With respect to sworn, <br />police personnel, up to a <br />eight dollars ($68.00) <br />enrollee. <br /> <br />"safety-member" <br />maximum of sixty- <br />per month per <br /> <br />2 . <br /> <br />with respect to non-sworn, <br />member" personnel, up to a <br />four dollars ($64.00) <br />enrollee. <br /> <br />"miscellaneous- <br />maximum of sixty- <br />per month per <br /> <br />Any contribution necessary to maintain benefits <br />under such long-term disability insurance plan <br />provided by the Association for covered employees <br />in excess of the amounts set forth as above shall <br />be borne by the Association and/or the enrollees. <br /> <br />D. Life Insurance. The City shall maintain in effect <br />for the term of this Agreement its existing life <br />insurance plan for employees covered by this <br />Agreement on the same basis as said plan was <br />offered to employees as of December 31, 1988 <br />except the term life insurance coverage for each <br />affected employee will be in the amount of twenty <br />thousand dollars ($20,OOO), plus twenty thousand <br />dollars ($20,OOO) accidental death and <br />dismemberment (AD + D) coverage, at no cost to the <br />employee. . <br /> <br />Retiree Health Insurance Plan. Members retiring on or <br />after July 1, 1997, will be provided health insurance <br />premium reduction assistance. <br /> <br />All bargaining unit employees and the Association shall <br />hold the City harmless, defend and indemnify the City for <br />any claims regarding the administration of, or the <br />payment of, claims under any Association designed retiree <br />health insurance benefit plan. <br /> <br />Employees on unpaid leave of more than 15 calendar days <br />shall not receive the Cityl s contribution toward any <br /> <br />50 <br />
