Description of Work - In the space below, describe the program to be funded during the 12-month contract period. <br />What specific activities will be undertaken during the contract period. Only the <br />viewable space will print. <br />The Linkages Program provides case management services and comprehensive information and referrals to frail <br />elderly and disabled persons aged 18 and above to aid in obtaining appropriate community resources that prevent or <br />delay institutionalization. To meet the needs of vulnerable adults living in Santa Ana, Linkages staff will: <br /> <br />J Provide phone intake services in person during business hours with bilingual service available. <br />:::J Respond promptly to requests for case management &/or service referral requests. <br />:::J Provides timely assessments and create an appropriate care-plan using an informal collaborative or contractual <br />approach. <br />= Purchase assistive devices and respite services for clients as care plan indicates. <br />= Refer caregivers to counseling, family consulting services and respite assistance. <br /> <br />Schedule of Performace <br />Estimate the number of ONLY unduplicated Santa Ana residents to be served by the funded program during the 12- <br />month contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
