When ready to leave the shelter the client is <br />assisted in finding permanent housing and is encouraged to return for outreach assistance as desirable and necessary. <br />the goal is to have every client that enters the shelter move to a permanent residence and self-sufficiency. The success <br />rate for this goal is typically 75%. <br />Thomas House works with Orange County Partnership and enters data into the HUD HMIS data base for every bed in <br />the shelter. <br /> <br />The HUD goat is to have every client move to a permanent residence and independent living <br /> <br />Exhibit A-1 <br />Page 3 of 3 <br />
