Please review the <br />agreement. Once reviewed please print 3 copies and sign all three on the signature page AND Exhibit C. After <br />signing all 3 copies please mail them to: <br /> <br />Frank Hernandez <br />City of Santa Ana <br />Community Development Agency, M-25 <br />20 Civic Center Plaza <br />Santa Ana, CA 92701 <br /> <br />These copies will then receive the final signatures to be executed. Once all the appropriate signatures have been <br />received an original copy will be sent back to your organization. Please return signed copies by 6/22/07 <br /> <br />In addition, in order to execute the agreement the City will need a current general liability certificate with additional <br />insured endorsement as well as a workers' compensation certificate. The following is information regarding the <br />current insurance policies on file by this office. <br /> <br />Gen. Liability 6/15/07 <br />WIC 1/1108 <br /> <br />If the insurances have expired please submit a current copy along with your agreement.
