Fletcher, City Attorney <br />From: Carolyn Richard, Claims Admini at r <br />Date: November 6, 2007 <br />Re: Workers' Compensation Destruction of Rec ds <br />In accordance with the citywide records retention adopted by the City Council and <br />amended by resolution number 2006-045 in July, 2006, attached is an alphabetical <br />list of indemnity and medical only claim files for 22 boxes of files ready to be <br />destroyed. These files are at least 5 years old and/or are beyond the statute of <br />limitations. We have no more room to house these 423 claim files. This is only a <br />partial listing of files from A through G, and we will commence another destruction <br />project next year. <br />Please authorize the destruction of these records without need for microfiche.
