City of Santa Ana <br />Scope of Work <br />Name of Organization Feedback Foundation <br />Name of Funded Program Home. Delivered Meal Program <br />Annual Accomplishment Goal <br />I. Total number of unduplicated clients (Santa Ana and Non-Santa Ana Residents) anticipated to be served by the <br />funded program, named above, during the 12-month contract period. <br />1704 Persons <br />II. Number of ONLY unduplicated Santa Ana residents to be served by the funded program, named above, during the <br />12-month contract period. <br />420 Persons <br />Program and Funding Description <br />III. Description of Work - In the space below, describe the program to be funded during the 12-month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewaoie space ww pant. <br />Feedback Foundation seeks to offer older adults and their families a variety of supportive services within their diverse <br />communities designed to enhance their overall independence, well-being, and dignity. The donation based home <br />delivered meal program assists frail, at-risk older adults, to remain in their home following the onset of a chronic or <br />acute physical and/or mental debililitating illness. Three meals are prepared and delivered to the elder's home <br />Monday through Friday. Meals can be delivered for a short time, such as following a surgery, or it may be for an <br />indefinite time supporitng an elder and helping them remain independent. The Home Delivered Meal Program strives <br />to enhance the physical and mental well being of each homebound elder. Under the supervision and guidance of our <br />Registered Dietician, all nutritional needs are addressed. The daily connections with the delivery drivers and Case <br />Managers allow other needs to be addressed as well. Case Managers work directly with the clients on special <br />problems or needs such as medical insurance, transporation, legal problems, unmet medical needs, personal hygiene <br />'and homemaking needs, caregiver assistance, medication management, etc. There is always a watchful eye on all <br />recipients and many receive additional support from the community agencies and service clubs that we partner with <br />which ultimately reduces feelings of isolation and loneliness. <br />Schedule of Performace <br />Estimate the number of ONLY unduplicated Santa Ana residents to be served by the funded program during the 12- <br />month contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
