V. PROPOSAL FORM <br />Responses to this Request for Proposal are due at 5:00 p.m. FRIDAY, NOVEMBER 9, 2007. The <br />undersigned Vendor agrees to provide carnival services at City -owned parks in accordance with the <br />specifications. I/We have stated herein the services and fees that Uwe will furnish and deliver as <br />specified. <br />Award shall be based upon the evaluation criteria included in Section III.
