Po~icC~ (~-~ <br />~}y 9n~ray THIS SIXTH AMENDMENT TO AGREEMENT is entered into this7`h day of <br />April, 2008, by and between Vet Care Vaccination Services, Inc., a California <br />corporation ("Consultant") and the City of Santa Ana, a charter city and municipal <br />corporation of the State of California ("City"). <br />Recitals: <br />A. The parties entered into Agreement #A-2001-102, dated May 7, 2001, (hereinafter <br />"said Agreement") by which Consultant has provided low cost dog and cat <br />vaccination clinics to serve City residents. <br />B. In accordance with the terms and conditions of said Agreement, the parties wish to <br />renew said Agreement for an additional one-year period. <br />Wherefore, in consideration of the covenants contained in said Agreement, and subject <br />to all the terms and conditions of the Agreement, except those amended in this Sixth <br />Amendment to Agreement, the parties agree as follows: <br />1. Consultant shall continue to provide low cost pet vaccination services to the residents <br />of the City of Santa Ana, at the rates and charges set forth in Exhibit A-1 to this <br />Amendment, incorporated by reference. Consultant shall reimburse a portion of the <br />receipts to City far its efforts in advertising the clinics, as set forth in said Agreement. <br />2. Section 4, TERM, shall be amended to extend the term through June 30, 2009. <br />IN WITNESS WHEREOF, the parties hereto have executed this Sixth Amendment to <br />Agreement on the date and year first written above. <br />ATTEST: r <br />.V <br />'~/ Patricia E.
