Also, <br />prior to the younster's departure, esp will gather information about their future living conditions and stability, whether the <br />youth will be reunified with their families or referred to alternative placements. Through follow-up phone calls, esp will <br />confirm the youth's placement and level of appropriate living situation. esP's tracking will provide the City of Santa Ana <br />with information on how many youth move back to their homes and how many transition into alternative placements. <br /> <br />Exhibit A-1 <br />Page 3 of 3 <br />
