Total number of unduplicated clients (Santa Ana and Non-Santa Ana Residents) anticipated to be served by the <br />funded program, named above, during the 12-month contract period. <br />55 Persons <br />II. Description of Work - In the space below, describe the program to be funded during the 12-month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewaoie space wm <br />Type in description here. <br />The Infant Family Focus program provides direct services to blind, visually impaired, and multi-impaired babies, <br />birth to three years of age. Infant development specialists and occupational therapists make home visits and work <br />with the baby and family to develop and implement the best possible program for the baby. <br />Parents take a vital role in their baby's development as they learn pratical skills to help their baby learn in ways <br />other than visual. They help their baby explore and learn about their world. The family is encouraged to learn all <br />they can about their baby and are invited to meet with other parents. They learn about other resources in the <br />community to help them. <br />Funding from Santa Ana's CDBG program will help pay salaries of the staff directly involved with serving these <br />families. <br />Schedule of Performace <br />Estimate the number of ONLY unduplicated Santa Ana residents to be served by the funded program during the 12- <br />month contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
