A hotline <br />call form is completed by the hotline staff for each call. The call form is designed to gather the following <br />information: date & time of the call; caller's immediate situation - need for shelter or other supportive <br />services; demographic data such as name, age, gender, race, ethnicity, household size, income level; and the <br />disposition of the call listing referral resources the caller was linked to for services sought. <br />The date is intered into DVTRAK data base from which a monthly spreadsheet is created in excel. From this <br />data we can create a summary of the data on each call received. This information is then readily available for <br />providing statistical data to program directors and funders for review and verification of services provided. <br />Exhibit A-1 <br />Page 1 of 1 <br />
