<br />CITY COUNCIL MEETING DATE: <br /> <br /> <br />REQUEST FOR <br />COUNCIL ACTION <br /> <br />CLERK OF COUNCIL USE ONLY: <br /> <br />JULY 7, 2008 <br />TITLE: <br />AGREEMENT FOR <br />COMPREHENSIVE HAZMAT/SWAT <br />PHYSICAL EVALUATIONS <br /> <br />APPROVED <br /> <br />o As Recommended <br />o As Amended <br />o Ordinance on 1 sl Reading <br />o Ordinance on 2nd Reading <br />o Implementing Resolution <br />o Set Public Hearing For <br /> <br />dUlZ <br /> <br />CITY MANAGER <br /> <br />CONTINUED TO <br /> <br />FILE NUMBER <br /> <br />RECOMMENDED ACTION <br /> <br />Authorize the City Manager and Clerk of the Council to execute the <br />attached agreement with Pulmonary Consultants and Primary Care Physicians <br />Medical Group, Inc., subject to non-substantive changes approved by the <br />City Manager and City Attorney, in the amount of $30,000. <br /> <br />DISCUSSION <br /> <br />In August 2007 City Council approved an agreement with Pulmonary <br />Consul tants & Primary Care Physicians Medical Group for annual <br />HazMat/SWAT physicals, in the amount of $30,000. These physicals detect <br />the general health condition of the employee and the increased risks of <br />respiratory and heart disease linked to exposure to hazardous materials. <br />The Fire and Police Departments are requesting amending this agreement by <br />$30,000, for an aggregate amount not to exceed $60,000. <br /> <br />FISCAL IMPACT <br /> <br />Funds are budgeted in the fiscal year 2008-09 Fire Department, Fire <br />Suppression, Other Contractual Services account (account no. 11-323-6291, <br />$20,000); and the Police Department, Human Resources Division, Other <br />Contractual Services account (account no. 11-332-6291, $10,000). <br />APPROVED AS TO FUNDS AND ACCOUNTS: <br /> <br />{)./vOA '-- ~----=::.. <br /> <br />Marc Martin <br />Fire Chief <br />Fire ,partment <br /> <br />~C2..-~ <br /> <br />Paul M.
