As they come to the end of their stay in our program, housing is addressed in their weekly meetings with <br />the Financial Empowerment Manager (FEM.) The discussion about housing includes two major considerations: <br />what the resident is able to afford and if they are able to live on their own in an apartment, etc. For those who are <br />ready to live on their own, the FEM assists them in finding a safe, affordable place in an apartment, a room in a <br />large home, etc. If they aren't ready to live on their own, they are referred to other, longer-term transitional housing <br />programs throughout the County. <br />In addition to weekly meetings with the FEM, every resident also meets with her Case Manager on a weekly basis. <br />Notes of these meetings are kept in each resident's files as well as entered into the CMTS (Client Management <br />Information System.) When a resident leaves our program, her forwarding address, location of her job and her <br />monthly income are entered into her file, both paper as well as CMTS. All program graduates are invited to <br />partiicpate in our monthly Alumni Support Group. Finally, each program graduate will receive a survey, after she <br />has been out of the program for one year, to track any changes in her job, monthly income, home address, etc. <br />txnldlt A-1 <br />Page 1 of 1 <br />
