City of Santa Ana <br />Scope of Work <br />Name of Organization Women's Transitional Living Center, Inc. (WTLC) <br />Name of Funded Program Emergency Shelter Programs <br />Annual Accomplishment Goal <br />I. Total number of unduplicated clients (Santa Ana and Non -Santa Ana Residents) anticipated to be served by the <br />funded pro ram, named above, during the 12 -month contract period. <br />750 Persons <br />II. Number of ONLY unduplicated Santa Ana residents to be served by the funded program, named above, during the <br />12 -month contract period. <br />200 Persons <br />Program and Funding Description <br />III. Description of Work - In the space below, describe the program to be funded during the 12 -month contract period. <br />What specific activities will be undertaken during the contract period. Only the <br />viuvvaulu aNa%.c will pill n. <br />The Emergency Shelters Program includes the 45 -day program, the 90 -day program (for clients with addiction <br />issues), and the Safety Net program (the emergency motel shelter program). The overall purpose of the <br />emergency shelter programs is to assist victims of domestic violence/human trafficking in becoming safe, self- <br />sufficient members of society. <br />Objectives: <br />1) Shelter and safety: at least 200 adults and children from the City of Santa Ana will be provided shelter in one <br />of the two emergency shelter and will receive supportive services through the programs to include: case <br />management/advocacy, transportation, translation, 24/7 hotline, children's programs, educational classes, <br />process/empowerment groups, counseling, substance abuse treatment, human trafficking assistance, <br />legal/immigration/social service advocacy, safety/independence planning, on-site medical screening and care <br />and food/clothing/toys. <br />2) Increasing financial independence: at least 85 adults from the City of Santa Ana will receive career guidance, <br />participate in work experience programs, receive job training/placement assistance, and/or being the process of <br />becoming documented so they con obtain higher paying jobs, apply for jobs, and/or increase their level of <br />income. <br />3) Increasing emotional stability: at least 165 adults and children from the City of Santa Ana will receive <br />counseling (individual, group, family, crisis, children's) on a weekly basis in an effort to increase their emotionaly <br />stability. <br />Schedule of Performace <br />Estimate the number of ONLY unduplicated Santa Ana residents to be served by the funded program during the 12 - <br />month contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
