As they come to the end of their stay in our program, housing is <br />addressed in their weekly meetings with the Financial Empowerment Manager (FEM.) The <br />discussion about housing includes two major considerations: what the resident is able to afford <br />and if they are able to live on their own in an apartment, etc. For those who are ready to live <br />on their own, the FEM assists them in finding a safe, affordable place in an apartment, a room <br />in a large home, etc. If they aren't ready to live on their own, they are referred to other, longer- <br />term transitional housing programs throughout the County. <br />In addition to weekly meetings with the FEM, every resident also meets with her Case <br />Manager on a weekly basis. Notes of these meetings are kept in each resident's files as well <br />as entered into the CMTS (Client Management Information System.) When a resident leaves <br />our program, her forwarding address, location of her job and her monthly income are entered <br />into her file, both paper as well as CMTS. All program graduates are invited to partiicpate in <br />our monthly Alumni Support Group. Finally, each program graduate will receive a survey, after <br />she has been out of the program for one year, to track any changes in her job, monthly <br />income, home address, etc. <br />Exhibit A-1 <br />Page 3 of 3 <br />
