Description of Work - In the space below, describe the program to be funded during the 12 -month contract period. <br />What speck activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print. <br />SMEDA's eviction prevention program is aimed at ensuring the our clients' basic need for shelter is met. Our <br />Process entails a thorough intake assessment of the clients' needs to determine the appropriate course to follow so <br />that they gain or retain self sufficiency. We will attempt to address their immediate needs first. in doing so, we <br />attempt to gain the clients' trust that we have their best interest at heart. During the course of the interview we try to <br />ascertain a clear picture of all the clients' needs, all the while giving them an opportunity to fully explain how they <br />arrived at the current crisis situation. If, during the course of the interview, we learn that a client not only needs <br />assistance to avoid eviction, but also with food, clothing or any other basic need, we will either offer the services wej <br />have available or refer him/her to the appropriate agencies or organization that could offer assistance. <br />The interview entails a complete discussion of how the client got to this specific level of need; i.e., why he/she is <br />facing eviction and what avenues the client has to ensure that he/she will not be facing the same crisis in the <br />following month. We make every effort, within our capacity, to ensure that the client will not face similar crisis in the <br />near future. <br />Within the six months following service, we conduct a follow-up contact to determine the client's status and the <br />effectiveness of our service to him/her. <br />scnecuie of Performace <br />Estimate the number of ONLY unduplicated Santa Ana residents to be served by the funded program during the 12 - <br />month contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
