City of Santa Ana <br />Scope of Work <br />Name of Organization -Orange County Bar Foundation <br />Name of Funded Program Stop Short of Addiction -Spanish Program <br />Annual Accomplishment Goal <br />I. Total number of unduplicated clients (Santa Ana and Non -Santa Ana Residents) anticipated to be served by the <br />funded pro ram, named above, during the 12 -month contract period. <br />l 20J Persons <br />II. Number of ONLY unduplicated Santa Ana residents to be served by the funded program, named above, during the <br />12 -month contract period. <br />60 Persons <br />Program and Funding Description <br />III. Description of Work - In the space below, describe the program to be funded during the 12 -month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />Stop Short of Addiction's Spanish services target at -risk Latino youth , ages 12 to 17,that have issues with <br />substance abuse and other delinquent behavior. Parental participation is required. PROGRAM ACTIVITIES <br />include providing with the following intervention services to youth and family: <br />1) CLINICAL INTAKE ASSESSMENT- 2 hours <br />2) THREE PROGRAM SESSIONS- 12 hours total <br />3) FAMILY THERAPY -on- hour sessions for up to 12 weeks, as needed and <br />4) Our CASE MANAGEMENT SERVICES are provided as needed to link youth to local agencies that provide <br />additional behavioral health services. <br />ocneouie or rertormace <br />Estimate the number of ONLY unduplicated Santa Ana residents to be served by the funded program during the 12 - <br />month contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
