5/05 <br />er documentation supporting and evidencing how the ESG <br />FUNDS have been expended during the applicable quarter. <br />(ii) Quarterly Activity Report: Including in such reports <br />evidence of compliance with all applicable provisions of Title X <br />and the LBP Regs, in particular Subpart K and, as applicable <br />Subpart J, as further described in subsection (d) hereinafter, and <br />Subpart M, if SUBRECIPIENT provides long term tenant -based <br />rental assistance. SUBRECIPIENT agrees to provide CITY with <br />written cumulative (year -to -date) reports of its activities on or <br />before the 15th day of October, January, April and July for the <br />period beginning July 1, and through and including the previous <br />three -month reporting period setting forth the activities, program <br />accomplishments, new program information and year -to -date <br />program statistics on expenditures, caseload and activities. When <br />appropriate, pictures should be included. <br />(iii) Quarterly certification of match, plus documentation of match <br />source. <br />(iv) Any such other reports as CITY (or HUD) shall reasonably <br />require and /or request, including but not limited to the follow- <br />ing information: monthly records of all ethnic and racial statistics <br />of persons and families benefited by SUBRECIPIENT in the per- <br />formance of its obligations under this Agreement, including, but <br />not limited to, the activities, program accomplishments, new pro- <br />gram information, match fund certification and current program <br />statistics on expenditures, caseload and activities. When appropri- <br />ate, pictures /slides should be included. <br />(3) Recordkeeping of Ongoing Maintenance under the LBP Reas. If the services and <br />program activities provided by SUBRECIPIENT hereunder establish an ongoing relationship <br />with a property (such as long term tenant based rental assistance program [Subpart M] or transi- <br />tional or permanent housing operated or administered by the SUBRECIPIENT), then the <br />SUBRECIPIENT shall provide a report to the CITY evidencing that SUBRECIPIENT and /or the <br />property owner(s), as applicable, has performed ongoing maintenance of the subject property. <br />Ongoing maintenance shall include evidence of an annual clearance certification of the subject <br />residential property conducted by a DHS certified technician to ensure that LBP hazard reduc- <br />tion measures are maintained. All maintenance activities shall be conducted to ensure the threat <br />of LBP hazards is minimized. <br />IF <br />
