Description of Work - In the space below, describe the program to be funded during the 12 -month contract period. <br />What specific activities will be undertaken during the contract period. Only the <br />viewable <br />Type in description here. <br />The Foundation will - during the 2008-09 Fiscal Year provide the following services and activities:(1) Emergency <br />Dental Assistance. The Foundation will continue to fund emergency dental care for Santa Ana residents through <br />reduced cost services performed by Jay Bauman DDS, a Santa Ana dentist. Many participants are in desparate <br />need of services and would find it difficult to satisfy employment and/or educational requirements without <br />assistance. (2) Eye Care. The Foundation arranges free eye exams/corrective lenses through Lenscrafters "Gift of <br />Sight" program.(3) Transportation Vouchers. The Foundation will continue to provide Santa Ana residents with <br />OCTA bus passes to enable them to attend mandatory court/counseling/probation appointments as well as <br />school/work.(4) Opportunities to satisfy cultural/civic activities requirements. The Foundation will continue to offer <br />Santa Ana residents the opportunity, at no cost to the participant other than his/her transportation expenses, to <br />satisfy the requirement to engage in cultural, civic and educational activities such as specially designed tours of the <br />Getty Museum, the Laguna Canyon Wilderness Park and the Upper Newport Bay. (4) Link participants with existing <br />resources. The Foundation will continue to help link Santa Ana residentstparticipants with existing publicly and <br />privately funded agency programs such as Women Helping Women that provide clothing and assistance finding <br />employment and Working Wardrobes that assist with clothing and building self-esteem.(5) Form and support alumni <br />programs. The Foundation helps graduates form alumni groups and works with those groups to build membership, <br />become mentors to participants and help educate others. (6) Sponsor educational and employment oriented events <br />that will help participants identifies work opportunities before and after graduation.(7) Provide assistance to <br />participants that are required to pass the GED in order to graduate the Drug Court Program. <br />Schedule of Performace <br />Estimate the number of ONLY unduplicated Santa Ana residents to be served by the funded program during the 12 - <br />month contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
