C Ftfe i INSURANCE ON FILE; WORK MAY PROCEED UNTIL INSURANCE EXPIRES -i -cV GLERK OF,9OTck 2008 nATE' JUl 1 FIRST AMENDMENT TO AGREEMENT A- 2008 -183 THIS FIRST AMENDMENT TO AGREEMENT is made and entered into this 7th day of July, 2008 by and between the City of Santa Ana, a charter city and municipal corporation of the State of California ( "City ") and Pulmonary Consultants & Primary Care Physicians Medical Group, Inc. ( "Consultant "). RECITALS: 1. City and Consultant entered into Agreement - 2007 -192, dated August 20, 2007, (hereinafter "said Agreement "), by which Consultant has p rovided annual hazardous materials medical examinations for hazardous materials members pursuant to Title 8, Section 5144 of the CALOSHA Respiratory Protection Regulation. 2. In accordance with the terms and conditions of said Agreement, the parties wish to extend the term of said Agreement for an additional one -year period and increase compensation to pay for services during the extended term. WHEREFORE, in consideration of the mutual and respective covenants contained in said Agreement, and subject to all the terms and conditions of said Agreement, City and Consultant agree as follows: Section 2.a., COMPENSATION, shall be amended to add $30,000 to total compensation to pay for services during the extended term of said Agreement. 2. Section 3, TERM, shall be amended to extend the term of said Agreement through June 30, 2009. 3. Except as herein amended, all terms and conditions of said Agreement shall remain in full force and effect.
