INSURANCE NOT REQUIRED N-2004-133-04 <br />WORK MAY PROCEED <br />CLERK Of COUNCIL <br />DATE: ?- 2-3-0~ <br />FOURTH AMENDMENT TO CONSULTANT AGREEMENT <br />(~ : ~o~ic e.~~ <br />i 5 ~p_,uf1¢~tnO U / <br />J THIS FOURTH AMENDMENT TO CONSULTANT AGREEMENT is entered <br />into on June 30, 2008, by and between LETICIA QUIROZ ("Consultant") and the City of <br />Santa Ana, a charter city and municipal corporation of the State of California ("City"). <br />RECITALS: <br />A. The parties entered into Agreement N-2004-133, dated November 4, 2004 <br />(hereinafter "said Agreement") by which Consultant has provided teacher assistance <br />for jail inmate instruction in GED and computer classes. <br />B. In accordance with the terms and conditions of said Agreement, the parties wish to <br />extend said Agreement for an additional ten month term and increase compensation to <br />pay for services during the extended term. <br />WHEREFORE, in consideration of the covenants contained in said Agreement, and <br />subject to all the terms and conditions of said Agreement, except those amended in this <br />Fourth Amendment to Consultant Agreement, the parties agree as follows: <br />1. Section 2.a., COMPENSATION, shall be amended to increase compensation by <br />$10,000.00 to pay for services during the extended term. <br />2. Section 3, TERM, shall be amended to extend the termination date through June 30, <br />2009. <br />3. Section 5, INSURANCE, shall be deleted in its entirety. Due to the nature of services <br />provided, no insurance is required. <br />4. Except as herein amended, all terms and conditions of said Agreement shall remain in <br />full force and effect. <br />// <br />// <br />// <br />// <br />// <br />
