(Lifeskills, Involvement, Fitness, Education) <br />Annual Accomplishment Goal <br />I. Total number of unduplicated clients (Santa Ana and Non-Santa Ana Residents) anticipated to be served by the <br />funded ro ram, named above, during the 12-month contract period. <br />60 Persons <br />II. Description of Work - In the space below, describe the program to be funded during the 12-month contract period. <br />What specific activities will be undertaken during the contract period. Only the <br />viewable space will print. <br />Eastside L.I.F.E., with supervision/staff that includes 2 bilingual RN/PHN Nurses will engage 30 youth in the <br />Eastside Neighborhood of Santa Ana in learning life skills, community involvement and healthy living and will <br />structure the sharing of learned skills with their families for a total of 60 participants. Skills will be taught through <br />lessons adapted from the nationally recognized Why Try? curriculum and Leadership Bootcamps. <br />1. One time: Life Skills-Leadership and Character Development: This component will be taught utilizing a week- <br />long Leadership Bootcamp. Designed to teach team building, cooperation, positive personal interrelations, social <br />skills and a positive self-image, these Leadership Bootcamps will be contracted to Mark Tyler, former Marine and <br />retired Homeland Security, former Youth Pastor with proven track record in serving at-risk diverse youth. <br />2. 10 months-once per week. COMMUNITY INVOLVEMENT- will be taught through the structuring of activities <br />,that are built into classes focused on Asset Based Community Development. Participants will be taught the basi <br />'building blocks of Community Development through ABCD principals which focus on Community-Driven Agendas <br />for change. <br />3. 4 times per year: Grace will convene/join program parents and others in attending Eastside Neighborhood <br />Association meetings to encourage families to more actively participate in the life of their neighborhood . <br />4. 12 months-on-going: Fitness Components include: BMI, Fitness and Health education, Daily-7's-military <br />exercises, formation and discipline, and a neighborhood walking club <br />5. 10 months-once per week- Academic enrichment, homework club. <br />Schedule of Performace <br />Estimate the number of ONLY unduplicated Santa Ana residents to be served by the funded program during the 12- <br />month contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
