Total number of unduplicated clients (Santa Ana and Non -Santa Ana Residents) anticipated to be served by the <br />funded program, named above, during the 12 -month contract period. <br />2200 Persons <br />II. Number of ONLY unduplicated Santa Ana residents to be served by the funded program, named above, during the <br />12 -month contract period. <br />255 Persons <br />Program and Funding Description <br />III. Description of Work - In the space below, describe the program to be funded during the 12 -month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print. <br />Community SeniorSery seeks to offer older adults and their families a variety of supportive services within their <br />diverse communities designed to enhance their overall independence, well-being, and dignity. The donation <br />based home delivered meal program assists frail, at -risk older adults, to remain in their home following the onset <br />of a chronic or acute physical and/or mental debililitating illness. Three meals are prepared and delivered to the <br />elder's home Monday through Friday. Meals can be delivered for a short time, such as following a surgery, or it <br />may be for an indefinite time supporitng an elder and helping them remain independent. The Home Delivered <br />Meal Program strives to enhance the physical and mental well being of each homebound elder. Under the <br />supervision and guidance of our Registered Dietician, all nutritional needs are addressed. The daily connections <br />with the delivery drivers and Case Managers allow other needs to be addressed as well. Case Managers work <br />directly with the clients on special problems or needs such as medical insurance, transporation, legal problems, <br />unmet medical needs, personal hygiene and homemaking needs, caregiver assistance, medication <br />management, etc. There is always a watchful eye on all recipients and many receive additional support from the <br />community agencies and service clubs that we partner with which ultimately reduces feelings of isolation and <br />loneliness. <br />Schedule of Performace <br />Estimate the number of ONLY unduplicated <br />Santa Ana residents to be served by the funded program during the 12 - <br />month contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
