Orange County Conservation Corps <br />Santa Ana Workforce Investment Board <br />Request for Proposal <br />The Youth Service Provider Network <br />PY 2008/2009 <br />AVAILABILITY OF FUNDS: <br />1. Incentives are delivered contingent upon availability of funds and given on a first <br />come, first serve basis. <br />2. Incentives should not exceed a value of $40.00 per completed component. <br />Participant will have the option of choosing a gift card from a specific category <br />(i.e., restaurant, entertainment, retail, etc.) <br />Incentives <br />1. NA/AA Meetings -Must attend four (4) meetings at the OCCC. One time incentive only. <br />2. Skills Tutor -Participants whom have not passed the California High School Exit Exam <br />(CASHEE) must complete a total of 4 hours of skills tutoring per month. If participant <br />has completed all required credits and is only pending the CASHEE, participant must <br />complete a total of 8 hours per month. One time incentive only. <br />3. 9th Grade Skill Level - If found basic skill deficient during initial TABE assessment <br />participant must increase reading and math to a 9th grade level on TABE test. One time <br />incentive only. <br />4. Read Book -Must read a 100-page book with progress being monitored by participant's <br />Teacher or Case Manager. Some exceptions may be made for books less than 100 pages <br />such as the Ed De Jesus workbook "Makin It", etc. One time incentive only. <br />5. Complete Academic Subject -Must complete an assigned academic subject such as <br />English, Math, History, or Science. Academic subject must be at least 10 credits. One <br />time incentive only. <br />6. Complete a lob Shadow -Must job shadow an OCCC employee or any other employer <br />for a day in their career of interest. Participant must submit a typed 5 paragraph essay <br />on what they learned. One time incentive only. <br />7. Write an Autobiography -Must complete a typed 10 paragraph autobiography. One <br />time incentive only. <br />8. Keep a Personal Journal -Must maintain a weekly journal for a total of 12 weeks. <br />Journal entries should be at least one page per week. One time incentive only. <br />Corpsmember Bonus Plan <br />1. $60 Orientation Bonus (pass handbook, safety, tool test, earn CPR/First Aide Certificate, <br />100% attendance) <br />2. $50 Attendance Bonus (have a 100% attendance in any given month) <br />3. $40 Referral Bonus (refer someone to the OCCC who is hired and remains employed for <br />at least 30 days) <br />4. $50 Certification Bonus (chainsaw, chipper, forklift, and obtaining an ROP/Vocational <br />Certification) <br />5. $50 Drivers License Bonus (CA class Cand/or B) <br />6. $100 CASHEE Bonus (pass the CA High School Exit Exam) <br />7. $200 Education Bonus (earn high school diploma) <br />8. $30 Life Skills Bonus (Life skills packet/on-line training) <br />9. $20 Commendation Bonus (Receive a commendation letter from a project sponsor. <br />10. $ 50 Transition Bonus (Complete OCCC transition class components) <br />Page 13 of 24 <br />
