Describe how you will monitor program activity and manage for performance. <br />Peer mentors will meet regularly with each participant and assess progress on <br />program goals. They will also record program activity and participant performance in <br />client case files and the VOS database. Peer mentors will provide services based on <br />individual needs. The Foster Youth Liaison Project Supervisor will meet with peer <br />mentors on a regular basis to discuss participant progress, review case files and <br />ensure contract compliance. Also, WIA staff may perform program audits by <br />interviewing peer mentors and reviewing case files and may suggest changes to better <br />meet participant progress and contract obligations. <br />4. Describe how applicant plans to deliver academic assistance to raise the skill level of <br />youth who are basic skill deficient. <br />Peer mentors will deliver academic assistance in one or all of the following options. <br />• Peer mentors may provide tutoring to participants. They may utilize a homework <br />binder provided by WIA containing written resources and assignments for math <br />and reading <br />• Peer mentors may refer participants to outside tutoring resources when needed <br />• Peer mentors may work with participants utilizing web -based tutoring programs <br />5. Will the participant receive a certificate of completion or accomplishment for <br />participation? Will the certificate count for WIA credential attainment rate? <br />A certificate of completion will be provided for general participation in the Foster <br />Youth Liaison Project (FYLP), but will not count for WIA credential attainment. The <br />FYLP certificate of completions will simply serve as a form of recognition for <br />program completion. However, some participants may receive certificates of <br />completion for programs they may participate in. (dependent on individual's needs <br />and goals). Some certificates of completion may include a diploma, GED, higher <br />education degree or certificate or a completion certificate from ROP, in which case <br />would count for WIA credential attainment. Other certificates may not count for <br />WIA credential attainment. <br />6. How will continuous improvement be measured and conducted? <br />For participants who are determined to be Basic Skills Deficient, we will gage <br />improvement with the utilization of the Test of Adult Basic Education (TABE). The <br />TABE will be used as a pre and post test assessment to show literacy/numeracy gains. <br />The peer mentors will also gauge improvement by participant's competency during <br />the tutoring process. Furthermore, peer mentors will continually assess participant's <br />progress and success with individual and program goals. OCF will seek participant <br />feedback to assess program strengths and areas for improvement. Finally, SAWC <br />staff may review case notes, service provision, and data recording periodically <br />throughout the program year and may provide constructive feedback. Adjustments <br />will be made accordingly. <br />D. Program Description <br />1. Describe the overall services that will be made available to the WIA participant. <br />The Foster Youth Liaison Project will provide universal services onsite at the <br />Orangewood Resource Center. All foster/emancipated youth will receive universal <br />OCF/Foster Youth Liaison Project 5 <br />
