What services will be offered offer -Dull exits the program? Students will continue to receive <br />follow-up services with the Transition Specialist for a minimum of one year after they exit the <br />program. The Transition Specialist and participant v7ill work together in a documented. goal <br />oriented. youth -centered program that extends from recruitment through follow-upiretention. <br />Follow-up services will be based on the needs of the individual and may include: supportive <br />services: regular contact with the youth's employer to address work -related issues: ongoing job <br />skill coaching: development of time management skills: problem -solving: academic counseling <br />and development of an Individual Service Plan: and tracking the progress of youth in their <br />semester classes. Follow-up services will also include development of soft shills such as: critical <br />s�kin�. decision -making: priority setting: professionalism: and positive <br />thiinteraction with co- <br />workers. supervisors. colleagues. and instructors. <br />The Transition Specialist will coordinate career speaker events and fieldtrips to college <br />ill introduce students to professionals who possess the <br />campuses and workplaces. These events w <br />skills and knowledge that match students' interests and facilitate discussions on a variety of <br />available college�career options. The Transition Specialist will pmvide college/career counseling <br />during program enrollment and offer a series of follow-up workshops. These workshops may <br />include: development of marketable work habits. conflict resolution. public speaking. and <br />professional management skills. All of these follow-up workshops will be offered during one-on- <br />one sessions or in small Wm IA aluni groups to increase job satisfaction and retention. Parents will <br />also be invited to participate in workshops and recognition events to support their child's progress. <br />The Transition Specialist will initiate frequent contact with parents and participants to monitor <br />continued progress toward their goals and to address any new obstacles, quest' as they occur. <br />The Transition Specialist will provide at least one documented substantial service each month for <br />"exited" participants during the I -month follow-up period. <br />C. Goals! Objectives and Performance Levels <br />I. How will service(s) be deemed necessary? List tool(s) to be used for assessment o <br />participant's basic skill levels. Describe pre and post testing, prima► r and secon.dar;r b <br />oal <br />determination. All students will be referred to the program by teachers and staff that are most <br />familiar with the participant's goals and needs. During their first meeting. the Transition <br />Specialist will check youth's eligibility for enrollment in the College%Career Transition Program <br />and perform an objective assessment of the academic skills and inunediaey of service needs. <br />The Transition Specialist will use the infonnation obtained from the teachen'staff referral and <br />student's input to design the Individual Service Plan with the student. including their primary <br />and secondary goals. Following the Individual Service Plan design. the Transition Specialist will <br />use the Testing Adult Basic Education (TABS) test to assess the student's basic skills levels. <br />The testing w111 take place during the first or second one-on-one session will the Transition <br />Specialist. The TABS exam will be used for the post-test as well: students will take the TABS <br />test for the second time six months post -exit to assess for improvements in basic skills levels as <br />prescribed by federal performance measures. <br />2. How will services aid in meeting the T414 performance goals? The Transition Specialist will <br />provide job skill workshops during one-on-one sessions and/or small group workshops to <br />increase employability. These workshops will also increase the number of students securing <br />employment to meet the first WIA performance measure. In addition. the Transition Specialist <br />
