Total number of unduplicated clients (Santa Ana and Non -Santa Ana Residents) anticipated to be served by the <br />funded pro ram, named above, during the 12 -month contract period. <br />516 Persons <br />II. Number of ONLY unduplicated Santa Ana residents to be served by the funded program, named above, during the <br />12 -month contract period. <br />88 Persons <br />Program and Funding Description <br />lll. Description of Work - In the space below, describe the program to be funded during the 12 -month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />,iewaoie space win prim. <br />The program serves Orange County residents living with HIV who are at or below 150% of the federal poverty level <br />with some type of disability. The overall goals of the ASF Nutrition Services Program are to: 1) Prevent hunger; 2) <br />Provide high-quality nutritionally rich food; 3) Enhance the effectiveness of medical treatments; and 4) Serve as a <br />gateway to other services to promote independence and self-sufficiency. The Food Pantry was created to meet the <br />special needs of people living with HIV/AIDS on low incomes. By providing a variety of safe, easily prepared, <br />digested and nutritionally dense foods, including fresh produce. ASF seeks to help clients maintain a healthy weight <br />and promote their overall well-being. Each client, referred by a case manager, will receive food from the Food <br />Pantry as long as he/she meets the low-income guidelines and is classified as disabled. Most of our clients visit the <br />Food Pantry twice a month to pick up groceries. Each grocery order has enough nutritious food to make <br />approximately 3 meals per day for 15 days. The pantry is evaluated twice a year by a licensed dietician in order to <br />meet the nutritional requirements of people living with HIV/AIDS. Client surveys are conducted annually to ensure <br />that food options are culturally appropriate, easy -to -prepare and preferred by clients. To further meet the needs of <br />clients beyond the provision of food, multi-vitamins and additional nutritional supplements such as Ensure are <br />available to assist clients in taking HIV medications. In the event that clients are too ill to come to ASF, groceries will <br />be delivered to them via the ASF Extra Mile Transportation Program and ASF volunteers. ASF also offers bus <br />passes to clients who do not have their own transportation that can be used to get to the Food Pantry to pick up <br />their groceries. <br />Schedule of Performace <br />Estimate the number of ONLY unduplicated Santa Ana residents to be served by the funded program during the 12 - <br />month contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
