Rez Director of Personnel City of Santa Ana City Hall Santa Ana, California Dear Mr. Rez: This will acknowledge receipt of your governing body's Resolution of Inten- tion to amend your contract with this system to include pre-membership service. We are sending to you the following documents in order to accomplish the proposed amendment: 1. Amendment to contract, in duplicate. Please sign and return two copies to this system. 2. Ordinance, Form 128, in duplicate, approving amendment to contract. This ordinance must be adopted by affirmative vote of two-thirds of the members of the governing body no earlier than twenty days after the adoption of the Resolution of In- tention. The number of days and publications are to be inserted in Section 3 of the form to comply with local require- ments. One copy is to be completed and returned to this system. 3. Certification of Final Action by the Governing Body, Form 130, (to be completed and returned to this system). Effective date of the amendment cannot be earlier than the first of the month following the effectivity of final action of your governing body. The amendment has been prepared using the agency contribution rates which will become effective on July 1, 1963. However, the current service rate has been increased by 0.150 to cover agency cost for the pre-membership service which you are now providing by this contract amendment.
