(8) A statement in writing, and dated, by the applicant that he or she certifies under penalt~of periury that all information contained in the application is true and correct (d) The chief of police shall have thirty (30) calendar days in which to investigate the application and background of the applicant. The chief of police or his representative shall render a recommendation as to the approval or denial of the permit to the city manager. (e) The city manager, after receiving the application and aforementioned recommendation, shall grant the permit if he finds in accordance with section 22-3(f)(1) through 22-3(f)(6), and in addition finds: (1) The applicant has furnished an acceptable medical certificate in compliance with this chapter. (46) The applicant sis at least eighteen (18) years of age. (f) Any person denied a permit by the city manager pursuant to these provisions may appeal to the city council pursuant to Chapter 3 of the Santa Ana Municipal Code. (g) It shall be the responsibility of any permit holder to notify the City of his or her home address as well as the address of any business establishment where he or she re ularly works as a massage technician whether as an employee or as an independent contractor A permit holder shall notify the City within thirt~30) days of changing either his or her home address or the address of the massage establishment where he or she re ug larly works as a massage technician.
