EXHIBIT A -1 <br />SCOPE OF SERVICES <br />Consultant will teach "How to Achieve Permanent Weight Loss and Health" classes for <br />participants 18 and over. The class will teach participants how to stabilize their weight, <br />eating and relationship with food. The low glycemic index eating plan can be followed <br />by everyone whether vegan, vegetarian, fish eater or omnivore. <br />The one day class will be held on Saturdays, and will be conducted approximately every <br />five (5) weeks. <br />Classes will be held on a day and time mutually agreed between Consultant and City. <br />Minimum participation is 8 maximum is 35. <br />Consultant will provide and be responsible for equipment, records, personnel and clean <br />up of the facilities and materials necessary to ensure the safety and effectiveness of <br />instruction. <br />If Consultant allows others to teach the class, any such teacher shall be over 21, have <br />obtained and maintain an instructor rating and be covered by the Consultant's insurance. <br />Consultant shall provide documentation to verify instructor and insurance requirements. <br />FEES <br />Each participant shall pay a $10.00 fee per class. A material fee, paid to the instructor, <br />will be collected at the commencement of the class. <br />No refunds will be made after the second class meeting unless class is cancelled by City. <br />City shall collect all registration fees. Consultant shall receive seventy percent (70 %) of <br />the total fees collected each month. City shall retain thirty percent (30 %) as an <br />administration fee. Consultant shall not waive class registration nor allow participation <br />by anyone not registered. <br />City may audit Consultant's records to insure compliance with this Agreement. <br />
