(13) T"-~;~Proof of ~ submission to the a~l+sa~t <br />+^ f~,~California Department of Justice a complete set of <br />fingerprints ~•~"°n ^°°~°~taken by a police department or <br />California Department of Justice approved contactor. The <br />applicant shall be responsible for payment of any <br />fingerprinting fee. <br />(14) A site plan depicting the building and/or unit proposed for the <br />massage establishment and a dimensional interior floor plan <br />depicting how the massage establishment will comply with all <br />applicable requirements of this chapter. <br />(15) The complete name, date of birth, phone number and <br />residence address of the proposed on-site manager who will <br />be principally in charge of the operation of the establishment, <br />and any other information as requested by the Massage <br />Establishment application. A manager must possess the same <br />qualifications as an owner. <br />(16) A statement in writing, and dated, by the applicant that he or <br />she certifies under penalty of perjury that all information <br />contained in the application is true and correct. <br />(d) The applicant, if an individual, or designated responsible managing <br />officer, if a partnership or corporation, shall personally appear at the police <br />department of the City of Santa Ana and produce proof that the required <br />application fee has been paid and shall present the application containing the <br />aforementioned and described information. <br />(e) The chief of police shall have thirty (30) calendar days in which to <br />investigate the application and background of the applicant. The chief of police or <br />his representative shall render a recommendation as to the approval or denial of <br />the permit to the city manager. The department of building safety and housing, <br />the fire department and the Orange County Health Department shall inspect the <br />premises proposed to be devoted to the massage establishment and shall make <br />separate recommendations to the city manager concerning compliance with the <br />foregoing provisions. The proprietor of such establishment shall pay a health <br />service fee as established by the County of Orange. Such fees shall be paid <br />directly to the county health officer to be retained by the county as <br />reimbursement for such inspection. <br />(f) The city manager, after receiving the application and <br />aforementioned recommendations, shall grant the permit if he finds: <br />(1) The required fee has been paid. <br />Ordinance No. NS-XXX <br />Page 8 of 36 <br />11 A-8 <br />
