(3) Proof of certification of passage of the required City of Santa Ana test. (4) The applicant must provide a valid and current CPR certificate issued by the American Red Cross or the American Heart Association. (5) The applicant has not had a massage establishment permit or a massage technician's permit or other similar license or permit denied or revoked for cause by this city or any other city located in or out of this state within +h° ften (10) years prior to the date of application. (46) The applicant eis at least eighteen (18) years of age. (f) Any person denied a permit by the city manager pursuant to these provisions may appeal to the city council pursuant to Chapter 3 of the Santa Ana Municipal Code. (g) It shall be the responsibility of any permit holder to notify the City of his or her home address, as well as the address of any business establishment where he or she regularly works as a massage technician, whether as an employee or as an independent contractor. A permit holder shall notify the City within thirty (30) days of changing either his or her home address or the address of the massage establishment where he or she regularly works as a massage technician. Section 9. Section 22-5.1 is added to Chapter 22 of the Santa Ana Municipal Code to read in full as follows: Sec. 22-5.1. Massage technician permit -renewal.
