INS'iRA =CE ON i~IIE N-2008-103-01 <br />~'dOF.K fv:aY FRCCEfD <br />UNTlL Ir~S'URANCE EXPIRES AMENDMENT TO AGREEMENT <br />/,~ -~3/~-~Gf.~~ <br />CLERK GF COL AMENDMENT, made and entered into this 1st day of June, 2009, by and between Sue Montelone <br />DATE: <br />(previously known as Sue McCullough) ("Consultant") and the City of Santa Ana, a charter city and municipal <br />~~9'~~ corporation duly organized and existing under the Constitution and laws of the State of California ("City"). <br />RECITALS <br />A. The City and Consultant entered into that certain Consultant Agreement dated July 24, 2008, hereinafter <br />referred to as "said Agreement", to provide job preparedness instruction for clients at the Santa Ana <br />WORK Center. <br />B. The parties hereto now desire to amend the "Compensation" term found in Section 2.a of said <br />Agreement. <br />WHEREFORE, in consideration of the mutual and respective covenants and promises hereinafter contained and <br />made, and subject to all of the terms and conditions of said Agreement as hereby amended, the parties hereto do <br />hereby agree as follows: <br />1. The "Compensation" section of said Ageement will be amended to add additional funds of Three <br />Thousand Two Hundred Dollars ($3,200) to read: <br />"...amount not to exceed Eight Thousand Two Hundred and no/100 Dollars ($8,200.00)." <br />2. Except as hereinabove modified, the terms and conditions of said Agreement remain unchanged and in full <br />force and effect. <br />IN WITNESS WHEREOF, the parties hereto have executed this Amendment to said Agreement the date and <br />year first above written. <br />ATTEST: <br />Patricia E.
