EXHIBIT A-2 <br />SCOPE OF SERVICES <br />1. Consultant shall provide gymnastics lessons which will teach basic gymnastics <br />movements including basic tumbling, body movement, floor exercises, and flexibility <br />training. Each class will include a warm up/stretching component and a cool down <br />component. <br />2. This class meets at Jerome Center Gym or Southwest Senior Center at a time and date <br />to be agreed between Consultant and City. <br />3. If Consultant allows others to teach his/her class, those teachers must be over 21, have <br />obtained and maintain an instructor rating, and be covered by Consultants' insurance. <br />Consultant shall provide City with documentation to verify instructor and insurance <br />requirements. <br />CLASS SIZE <br />1. Each Gymnastics class must have a minimum of 5 paid students and no more than a <br />maximum of 40 students. <br />2. If the minimum registration is not reached by the second class, the class may be <br />cancelled by mutual agreement of Consultant and City, without any compensation <br />owed to consultant. <br />CLASS FEES <br />1. Each class participant shall pay a registration fee of $30 per monthly session. <br />2. No refunds shall be made to participants after the first week of class unless the Parks, <br />Recreation, and Community Services Agency cancels the class. <br />3. The City shall collect the class fees from each participant during the registration <br />period. Consultant shall not collect fees, but shall refer all interested participants to <br />City for registration. <br />4. City agrees to pay Consultant seventy per cent (70%) of the total fees within fifteen <br />(15) working days after completion of the class session. City and Consultant agree <br />that City shall retain thirty per cent (30%) of the fees collected. <br />5. Consultant agrees that City representative shall be entitled to audit Consultant's <br />records to insure compliance with this Agreement. <br />6. Consultant may not waive class participation/registration fees. <br />7. City shall prepare class rosters and provide a copy to Consultant. Only registered <br />participants may participate in class. <br />
