Orange County Youth Commission <br />Santa Ana, California <br />Case Management/Intake Assessment: <br />We will be assigning this position to a New Employee the tutors will share intake <br />assessment responsibilities. Our Case Manger will administer original TABE <br />test. Our Case Manager will interview prospective youth to ensure that they meet <br />all WIA requirements for program entry. <br />Our case management supervisor will oversee a staff of tutors who provide <br />distinctive case management activities. Tutors will observe and note their <br />observations providing feedback to the Case Manager. Two tutors will be in <br />charge of case notes collected during the tutoring process. Notes are maintained <br />on academic progress or challenges, attitudes, behaviors, performance and any <br />other pertinent issues disclosed by the youth or observed. They will meet with the <br />Case Manager for one hour each week to share notes and develop intervention <br />plans, if necessary. <br />Throughout the course of the week, the case Manager has one-on-one and <br />group contact with all the youth involved in the program. He will keep detailed <br />record of attitudes, behaviors and performance on each youth involved in the <br />program. <br />Case management will provide documentation of the interventions and progress <br />in attaining academic or performance goals. Apost-test will be given when a <br />client exits the program. Periodic testing may be given after 6 months and 9 <br />months of the program to assess progress and provide continuous improvement <br />of the program design. Once the client exits the program, staff will continue to <br />follow up with the clients for 12 months by phone calls, counseling and invitation <br />to program activities. <br />The Case Manager, on a regular basis, will review each case record and develop <br />an individual case plan, accordingly. He will provide special counseling for <br />special needs. If a particular problem arises that falls outside Anthony's ability to <br />assess and intervene, he will refer to other appropriate counseling, <br />academic/career or social service programs. <br />Program Services/Parent and Employer lnvolvement~ <br />After assessing the needs of the individual participant, youth are introduced to <br />the various program opportunities at the Club. They are provided with an <br />overview of our resources, procedures, goals and expectations. They are <br />introduced to the staff, tutors and peers. They are presented with their Club <br />sweat-shirt to create a sense of belonging. We stress that staff are here to assist <br />them to reach their goals and are encouraged to communicate openly with staff <br />so we can partner with them. <br />14 <br />
