PART 1-B. ANNUAL PERFORMANCE UNDER THE ACTION PLAN <br />1. Summary of Housing Activities: <br />a. Supportive Housing Activities: During the 2008-2009 reporting period the following <br />supportive housing accomplishments were achieved through the expenditure of HOPWA <br />funds: <br />^ 12 individuals received treatment via the Medical/Social model residential detoxification <br />program. Program goal was 18. Accordingly, funding was adjusted during the final <br />months of the reporting period. <br />^ 16 individuals received supportive housing through the residential treatment for <br />substance abuse program, slightly more than the goal of 15 for the year (Gerry House). <br />This program received HOPWA funds for approximately 69% of its operating costs; <br />balance of funding is via Ryan White Care Act. The 16 HOPWA funded clients are in <br />addition to the 7 funded by Ryan White; in total, Gerry House served 23 unduplicated <br />clients. <br />b. Housing Assistance for the Homeless <br />^ 159 homeless individuals received transitional housing services. These clients include <br />those who were previously incarcerated, leaving substance abuse facilities, and those <br />living in motels or other temporary housing. <br />^ 47 individuals were provided with supportive housing for substance abusers (Start <br />House), 157% of initial goal of 30. <br />c. Other housing activities <br />^ 104 individuals/families received HOPWA-funded ~nergency financial assistance to <br />prevent homelessness, slightly abode (103/0) its ~oal of 101 far the program year. <br />^ 19 individuals/families received short-term rental assistance,€meeting its goal. <br />^ 325 individuals/families received housing coordination and linkage services, 41% more <br />than the goal of 230. <br />^ 180 individuals received life skills training, 150% of original goal. <br />^ 55 individuals/families were provided with permanent rental assistance. <br />2. Progress Evaluation: <br />Overall, the 2008-2009 HOPWA projects met or exceeded identified goals. Need for Social or <br />Medical Detox services did not meet the original goal (12 of 18, or 67%). This may be viewed as <br />success, as decreased need for these services among the HIV+ population is an overarching goal <br />of both the HOPWA program and the County of Orange's Health Care Agency. <br />Details on each projects goals and accomplishments follow: <br />52 <br />Draft 8/26/2009 <br />19F-58 <br />
