D. CONTINUUM OF CARE NARRATIVE <br />HUD has placed significant emphasis on meeting the needs of the homeless through a <br />Continuum of Care system. A Continuum of Care (CoC) is a comprehensive and coordinated <br />housing and service delivery strategy that responds to the different needs of homeless and at- <br />risk individuals and families, and helps them attain self- sufficiency. As a recipient of CDBG, <br />HOME, ESG and HOPWA funds, Santa Ana supported numerous public and nonprofit agencies <br />that reach out to the homeless, brings them into the system of care, and facilitates their <br />movement toward self- sufficiency. Actions undertaken during the 2008 -2009 report period are <br />outlined below. Detailed summaries of HUD - funded activities are also found in IDIS reports <br />incorporated into this performance report. <br />1. ACTIONS TAKEN TO ADDRESS THE NEEDS OF HOMELESS PERSONS & THE SPECIAL NEEDS OF PERSONS THAT ARE <br />NOT HOMELESS BUT REQUIRE SUPPORTIVE HOUSING (INCLUDING PERSONS WITH HIV/AIDS) <br />Santa Ana provided substantial CDBG, ESG and HOPWA resources to nonprofit agencies for <br />homelessness prevention, emergency shelter, transitional housing, and supportive services as a <br />means to implement the local and regional CDC. During the 2008 -2009 report period the City of <br />Santa Ana provided financial support to the following agencies that provided supportive <br />housing for homeless individuals and individuals with special needs: <br />■ Transitional Housing - Thomas House provided supportive services and transitional housing <br />for homeless families. Additionally, Mercy House and Wise Place provided transitional <br />housing and supportive services for single men, single women, women with children, and <br />persons with HIV /AIDS <br />■ Permanent Supportive Housing — No new permanent housing opportunities were created <br />for Santa Ana residents during the report period with CDBG, ESG or HOME funding; <br />however, the SAHA provided ten housing assistance vouchers to a local shelter provider to <br />assist clients graduating from transitional shelter access permanent housing. <br />22 Draft 8/26/2009 <br />19F -28 <br />
