.DURANCE NOT REQUIRED <br />WORK MAY PR~'!CEED <br />CLERK OF Ci~UNCiL <br />naT~: SAP 3 p 20Q.9 <br />ADDENDUM TO <br />ORANGE COUNTY PARTNERSHIP <br />HPRP AGREEMENT WITH <br />THE CITY OF SANTA ANA <br />A-2009-140 <br />c~ <br />-~ <br />~~ For purposes of this Grant and throughout the term of this Agreement with the City <br />~' of Santa Ana, the parties agree that due to the nature of the services being provided by <br />~~ ORANGE COUNTY PARTNERSHIP ("Subrecipient") to the CITY OF SANTA ANA, <br />~~-~ certain terms of the HPRP AGREEMENT must be revised as follows: <br />~' <br />.~ • Exhibit A will be the Proposal that was submitted by Orange County Partnership <br />. -- ~~ to the City. <br />~~ ~J <br />• Any language related to "reimbursement" will now be "fee for services" rendered. <br />• There is no "Budget" associated with the Agreement. The only requirement is <br />that Orange County Partnership does not exceed the grant amount of the <br />Agreement and the fee will be associated only with Data Collection. <br />• An estimated invoice schedule will be attached to the Agreement as Exhibit B <br />instead of the referenced "Budget". <br />• The only insurance requirements for Subrecipient will be to provide the City with <br />proof of Worker's Compensation insurance per California state law. <br />• There will be no accomplishment requirements other than providing Quarterly <br />Reports, Annual Reports and any other report(s) requested by the City or HUD. <br />• All other terms and conditions of the HPRP Agreement apply to Subrecipient as <br />set forth in said Agreement. <br />By its signature below, the Subrecipient agrees to the terms of this Addendum which is <br />attached to and incorporated into the HPRP Agreement. <br />Date: ~ 011 'Q°t <br />SUBRECIPIENT: <br />ORANGE COUNTY~,PARTNERSHIP <br />By: ~~ <br />Name: ~ ~~ Q <br />Title: ~ C~tlu-c ~1f~eC~cr <br />
