PROGRAM DESIGN 20 Points <br />HPRP specifically limits the use of program funds and the populations these funds can assist. <br />Describe the following aspects of the proposed program: <br />^ Identify the specific HPRP eligible activity proposed <br />^ Describe the readiness of the proposed program <br />^ Describe how at risk or homeless status of potential program beneficiaries will be determined and <br />documented <br />^ Describe planned community outreach efforts and how clients will be referred to the program <br />^ Describe how homeless persons or persons at risk of homelessness will be assisted to sustain self- <br />sufficiency incompliance with HPRP program regulations <br />^ Describe both current and proposed roles and responsibilities shared in collaborative partnerships <br />with other agencies that serve homeless persons <br />^ Describe how HPRP "Habitability Standards" and Lead-based Paint requirements will be met <br />^ Describe how Santa Ana residency be verified <br />Limit Response to 1,250 Words <br />OC Partnership seeks $50,000 funding for data collection services through CMIS from the City <br />of Santa Ana. As the lead agency for the county, OC Partnership provides all training, outreach, <br />and technical support for agencies using the system. <br />The City of Santa Ana, as the HPRP grantee, will be responsible for tracking and gathering data <br />from each city grantee to report the use of funds back to HUD. Sub-grantees must use CMIS to <br />report client progress as required by HPRP guidelines. In addition to the collection of the <br />universal and program data elements currently required by HUD, several new HPRP specific <br />fields must be captured, including: <br />• Grant identifier <br />• Homeless status <br />• Financial assistance provided <br />• Housing relocation/stabilization services provided <br />This data will allow Santa Ana the ability to more closely measure and report sub grantee <br />outcomes and effective use of funds. Quarterly and Annual Progress Reports to HUD will need <br />to be generated accurately. There are currently 34 service providers on CMIS. With this new <br />infusion of program funding more nonprofits will be required to contribute client data to CMIS, <br />moving our community closer to HUD's goal of a widely used regional homeless client database <br />that accurately paints a picture of homeless clients and services received in Orange County. <br />CMIS allows for an accurate regional and agency level view of our service delivery system, and <br />identification of service gaps required to inform local programmatic and funding decisions. Your <br />investment in CMIS will allow OCP to: <br />• Expand the CMIS system to meet the new HPRP data collection requirements <br />• Provide CMIS system setup, training and technical support to your sub grantee recipients <br />• Provide the data needed to complete the Annual Progress Report (APR) for sub grantees <br />to be submitted to the City of Santa Ana <br />RFP HOMELESSNESS PREVENTION & RAPID RE-HOUSING SERVICES 6/11/09 <br />10 <br />
