ADDENDUM TO <br />VOLUNTEER CENTER OF ORANGE COUNTY/ILLUMINATION FOUNDATION <br />HPRP AGREEMENT WITH THE CITY OF SANTA ANA <br />For purposes of this Grant and throughout the term of this Agreement with the City of <br />Santa Ana, the parties agree that the Volunteer Center of Orange County and the <br />Illumination Foundation (together the "Subrecipient") cannot separate or dissolve their <br />partnership without first sending a formal written request to the City for such separation <br />or dissolution and receiving prior formal written consent of the City by the City Manager <br />or his/her designee. <br />If they separate or dissolve their partnership, the City has absolute discretion to <br />recapture and reallocate the remaining balance of the Grant funds from the Volunteer <br />Center of Orange County and the Illumination Foundation to another qualified <br />Subrecipient. <br />By its signature below, the Subrecipient agrees to the terms of this Addendum which is <br />attached to and incorporated into the HPRP Agreement. <br />Date: 0V-19 <br />2S SUBRECIPIENT: <br />VOLUNTEER CENTER OF ORANGE COUNTY <br />By: <br />��,✓J- <br />Name: <br />Title: <br />IL TION FOUNDATION <br />By: cam. <br />Name: lbw ,; C` L e c) v, <br />Title:E� e 41 t J ' c <br />
