APPENDIX A Proposal Response Form <br />(Continued) <br />Sample Job #1: <br />Scope of Work: Provide and install (4) Category 5e cables, at 200 feet each, to the same <br />location in an air return ceiling area. Two cables will be terminated into acontractor- <br />provided 24-port patch panel on one-side and 8PSC-type Category 5e modules on the <br />station side; the other two cables will be terminated into acontractor-provided 300FT 110- <br />block assembly and again 8P8C-type Category 5e modules on the station side. Assume a <br />clear path in the ceiling to the patch panel and the station side, utilizing Erico J-hooks in <br />the ceiling for cable pathway support. On the station side, assume there is flex conduit <br />running from a single gang outlet box to the top of the wall, in a stub-out fashion. <br />1) Provide a breakdown of hours to be billed. <br />2) Provide a breakdown of cost of materials. <br />3) Provide details and amounts of any and all other charges that would be billed to the <br />City. <br />4) Provide approximation of elapsed time to complete work, beginning from authorization <br />to final acceptance. <br />Page 20 of 34 <br />
