EXHIBIT A <br />SCOPE OF SERVICES <br />1) Provider will teach a Modeling and Self- Improvement class a El Salvador Center. The class <br />will cover basic exercise and aerobics steps to keep you healthy and full of energy. <br />2) The classes will be held Wednesdays, 11 & up years of age at 3:30 pm starting November If G'� <br />and running monthly. <br />3) Provider will provide and be responsible for equipment, records, personnel and clean up of the <br />facilities and materials necessary to ensure the safety and effectiveness of said instruction. <br />4) If Provider allows others to teach her class, those teachers must by over 21 years of age, have <br />obtained and maintain an instructor rating, and be covered by Providers insurance. Provider shall <br />provide City with documentation to verify instructor and insurance requirements. <br />CLASS SIZE AND REGISTRATION <br />1) Each class must have a minimum of five paid students and no more than thirty. <br />2) No registration will be accepted after the second meeting of the class. <br />3) In the event the minimum number of enrollees is not realized by the second meeting, <br />FEES <br />1) Each participant shall pay a $45.00 class registration fee per session. <br />2) No refunds will be made to participants after the commencement of a session unless the class <br />is cancelled by the Parks, Recreation and Community Services Agency. <br />3) City shall collect registration fees from each participant. Provider shall refer interested <br />students to City for registration. <br />4) Provider shall receive seventy percent (70 %) of total fees collected. City shall retain thirty <br />percent (30 %) of the fees collected. <br />5) Provider agrees that City shall be entitled to audit Provider's records and classes to ensure <br />compliance with this Agreement and that all participants are registered. <br />6) Provider may not waive class participation/registration fees. <br />7) City shall prepare class rosters and provide a copy to Provider. Only registered participants, <br />listed on roster, may participate in class. <br />VA <br />
