EXHIBIT A SCOPE OF SERVICES 1) Provider will teach a Modeling and Self- Improvement class a El Salvador Center. The class will cover basic exercise and aerobics steps to keep you healthy and full of energy. 2) The classes will be held Wednesdays, 11 & up years of age at 3:30 pm starting November If G'� and running monthly. 3) Provider will provide and be responsible for equipment, records, personnel and clean up of the facilities and materials necessary to ensure the safety and effectiveness of said instruction. 4) If Provider allows others to teach her class, those teachers must by over 21 years of age, have obtained and maintain an instructor rating, and be covered by Providers insurance. Provider shall provide City with documentation to verify instructor and insurance requirements. CLASS SIZE AND REGISTRATION 1) Each class must have a minimum of five paid students and no more than thirty. 2) No registration will be accepted after the second meeting of the class. 3) In the event the minimum number of enrollees is not realized by the second meeting, FEES 1) Each participant shall pay a $45.00 class registration fee per session. 2) No refunds will be made to participants after the commencement of a session unless the class is cancelled by the Parks, Recreation and Community Services Agency. 3) City shall collect registration fees from each participant. Provider shall refer interested students to City for registration. 4) Provider shall receive seventy percent (70 %) of total fees collected. City shall retain thirty percent (30 %) of the fees collected. 5) Provider agrees that City shall be entitled to audit Provider's records and classes to ensure compliance with this Agreement and that all participants are registered. 6) Provider may not waive class participation/registration fees. 7) City shall prepare class rosters and provide a copy to Provider. Only registered participants, listed on roster, may participate in class. VA
