Use of CDBG Client Eligibility Certification has become an agency protocol for all recipients of the agency. A <br />standard "Homeowner Application" is utilized to secure and qualify potential service reciipients. Home ownership <br />and income are documented and verified. Throughout the year an ongoing Site Master "wait list" is maintained and <br />sites are assigned work activities and workscopes determined and manpower is coordinated. Work outcomes for <br />selected home rehabilitation units are tracked through a "Scope of Work & Task List." A "Work Completion" form is <br />presented to and signed by the homeowner. A file may be considered complete, however homeowners can continue <br />to contact the agency to address needs arising from changing circumstances in the home affected by physical or <br />social conditions.
