EXHIBIT A <br />SCOPE OF SERVICES <br />Consultant shall perform the following services for City personnel, as requested by City: <br />Post exposure TB screen (including any necessary X-rays) <br />Post exposure blood screen (including HIV, Hepatitis B & C) <br />PPD exam (including any necessary X-rays) <br />COMPENSATION <br />Consultant shall be compensated for each test administered as set forth in the schedule of <br />fees attached hereto. <br />Total compensation to be provided pursuant to this Agreement shall not exceed <br />$20,000.00 during the term of this Agreement. <br />
