EXHIBIT A <br />SCOPE OF SERVICES <br />1) Provider will instruct classes once a week in Beginning, Intermediate and Advanced <br />piano. Intermediate and advanced students must have more than one year of lessons. <br />Sessions shall be monthly. <br />2) Classes will be held Mondays as follows: 4pm to 5 pm-Beginner; Spm to 6pm- <br />Beginning & Intermediate; 6pm to 7pm-Advanced. <br />3) Provider will provide and be responsible for equipment, records, personnel and clean <br />up of the facilities and materials necessary to ensure the safety and effectiveness of said <br />instruction. <br />4) If Provider allows others to teach classes, those teachers must be over 21, hold a valid <br />instructor rating and be covered by Provider's insurance. Provider shall provide City <br />with documentation of compliance with the foregoing. <br />CLASS SIZE <br />1) Each class shall have a minimum number of 5 paid students and no more than 12. No <br />registration shall be permitted after the second meeting of the class. <br />2) In the event the minimum number of enrollees is not realized by the second meeting of <br />class, the class shall be cancelled. Provider will be under no obligation to provide <br />services and the City will be under no obligation to pay Provider compensation. <br />CLASS FEES <br />1) Each participant shall pay $30.00 class registration fee per session. Anticipated <br />revenue from this class shall be $10,000.00. <br />2) No refunds shall be made to participants after the second class meeting unless class is <br />canceled by the City. <br />3) City shall collect registration fees from participants during the registration period. <br />Provider shall not collect fees but shall refer interested participants to the City. City shall <br />prepare class rosters on provide copies to Provider. <br />4) Provider shall receive seventy percent (70%) of the total fees collected each month. <br />City shall retain thirty percent (30%) of the fees collected. Provider shall not waive fees. <br />6) The City shall be entitled to audit Provider's records to insure compliance with this <br />Agreement. <br />
