EXHIBIT A SCOPE OF SERVICES 1) Provider will instruct classes once a week in Beginning, Intermediate and Advanced piano. Intermediate and advanced students must have more than one year of lessons. Sessions shall be monthly. 2) Classes will be held Mondays as follows: 4pm to 5 pm-Beginner; Spm to 6pm- Beginning & Intermediate; 6pm to 7pm-Advanced. 3) Provider will provide and be responsible for equipment, records, personnel and clean up of the facilities and materials necessary to ensure the safety and effectiveness of said instruction. 4) If Provider allows others to teach classes, those teachers must be over 21, hold a valid instructor rating and be covered by Provider's insurance. Provider shall provide City with documentation of compliance with the foregoing. CLASS SIZE 1) Each class shall have a minimum number of 5 paid students and no more than 12. No registration shall be permitted after the second meeting of the class. 2) In the event the minimum number of enrollees is not realized by the second meeting of class, the class shall be cancelled. Provider will be under no obligation to provide services and the City will be under no obligation to pay Provider compensation. CLASS FEES 1) Each participant shall pay $30.00 class registration fee per session. Anticipated revenue from this class shall be $10,000.00. 2) No refunds shall be made to participants after the second class meeting unless class is canceled by the City. 3) City shall collect registration fees from participants during the registration period. Provider shall not collect fees but shall refer interested participants to the City. City shall prepare class rosters on provide copies to Provider. 4) Provider shall receive seventy percent (70%) of the total fees collected each month. City shall retain thirty percent (30%) of the fees collected. Provider shall not waive fees. 6) The City shall be entitled to audit Provider's records to insure compliance with this Agreement.
