EXHIBIT A -I <br />SCOPE OF SERVICES <br />I) Provider will teach "CrossFit Certitied Training, not your usual exercise class. <br />CrossFit training is for men and women who want core conditioning by building physical <br />ability in endurance, strength, flexibility, power, speed coordination, etc. Focus is on <br />making neurological or hormonal changes using multiple training methods, functional <br />movement and developing successful weight management strategies. Participants will <br />provide a mat and water and a set of heavy dumbbells. Class will be provided at <br />Thornton Park. <br />2) Classes will be held Mondays, Tuesdays and Thursdays for ages 18 and over from <br />6:30 to 7:30 pin. Session will be held every 4 weeks. <br />3) Provider will provide and be responsible for equipment, records, personnel and clean <br />up of the facilities and materials necessary to ensure the safety and effectiveness ofsaid <br />instruction. <br />4) if Provider allows others to teach classes, Chose teachers must be over 21, hold a valid <br />instructor rating and be covered by Provider's insurance. Provider shall provide City <br />with documentation of compliance with the foregoing. <br />CLASS SIZE <br />I) Each class shall have a minimum number of 8 paid students and no more than 35. <br />2) In the event the minimum number of enrollees is not realized by the second meeting of <br />class, the class may be canceled by the City. Provider will be under no obligation to <br />provide services and the City will be under no obligation to pay Provider compensation. <br />CLASS FEES <br />1) Each participant shall pay $119.00 class registration fee per session. Anticipated <br />revenue from this class shall be $ 10,000.00. A material fee shall be collected at the <br />beginning of each session and paid directly to the instructor. <br />2) No refunds shall be made to participants after the second class meeting unless class is <br />canceled by the City. <br />3) City shall collect registration fees from participants during the registration period. <br />Provider shall not collect fees but shall refer interested participants to the City. City shall <br />prepare class rosters on provide copies to Provider. <br />4) Provider shall receive seventy percent (70 1/16) ofthe total fees collected each month. <br />City shall retain thirty percent (30 %) of the fees collected. Provider shall not waive fees. <br />6) The City shall be entitled to audit Provider's records to insure compliance with this <br />Agreement. <br />
