ARRA EXHIBIT B <br />(Standard Agreement) <br />E. Close-Out Report <br />Subcontractor shall submit all the appropriate CAPOC close-out forms within <br />thirty (30) calendar days after the expiration date or when all funds have been <br />fully expended under this Agreement. Final reimbursement to Subcontractor, if <br />owed, shall be contingent upon receipt of the close-out packet by CAPOC and <br />subject to final review by CAPOC's Finance Department. <br />1) The close-out packet shall include the forms below which will be available <br />on the CAPOC website www.capoc.ora <br />a. CSD 925 Close out checklist with authorized signature <br />b. CSD 925 C Program Income/Interest Earned <br />C. CSD 925 D Equipment Inventory Schedule <br />2) Final expenditures must be submitted by entry onto the web-based <br />CAPOC system on or before the final report due date. <br />3) All adjustments must be submitted by entry onto the web-based CAPOC <br />System. Each adjustment must reflect the actual expenditure period when <br />the adjustment occurred. <br />5. ATTACHMENTS TO CONTRACT <br />A. Attachment I <br />Prior to execution of this Agreement by the State, Contractor shall submit to the <br />State Attachment I, CSBG Fiscal Data Series, which shall reflect a description of <br />projected spending for the contract term. <br />B. Attachment II <br />Contractor shall also submit to the State Attachment II, ARRA CSBG/NPI 901 <br />Program Report, which shall reflect a description of outcome goals for each year <br />of the contract term. This information will be used to monitor the success of the <br />identified National Performance Indicators relevant to its programs, activities, and <br />available resources for the delivery of services. Modifications to projected <br />outcomes will not be allowed without CSD approval. <br />C. Attachment III <br />Contractor shall also submit to the State Attachment III, ARRA CSBG/Project <br />Work Plan (CSD 920), which shall reflect a description of each project including <br />expected outcomes for the contract term. This information will be used to <br />monitor the success of the project. <br />ARRA-B4
