City of Santa Ana <br />23-HHAP-10015 <br />Page 14 of 24 <br />(2)The performance monitoring and accountability framework shall include <br />clear metrics, which may include, but are not limited to, the following: <br />(a)The number of individual exits to permanent housing, as defined by the <br />United States Department of Housing and Urban Development, from <br />unsheltered environments and interim housing result ing from this <br />funding. <br />(b)Racial equity, as defined by the council in consultation with <br />representatives of state and local agencies, service providers, the <br />Legislature, and other stakeholders. <br />(c)Any other metrics deemed appropriate by the council and developed in <br />coordination with representatives of state and local agencies, <br />advocates, service providers, and the Legislature. <br />(3)Data collection and reporting requirements shall support the efficient and <br />effective administration of the program and enable the monitoring of <br />jurisdiction performance and program outcomes. <br />Data shall include progress towards meeting the grantee's outcome goals. <br />If significant progress toward outcome goals has not been made, the <br />applicant shall: <br />(a} Submit a description of barriers and possible solutions to meet those <br />barriers <br />(b)Accept technical assistance from Cal ICH <br />(c)Include the progress towards outcome goals in all subsequent <br />quarterly reports, until significant progress is made as deemed by Cal <br />ICH •• <br />ii)Expenditure Report: The expenditure report shall contain data on <br />expenditures of HHAP-4 funding including but not limited to obligated funds, <br />expended funds, and other funds derived from HHAP-4 funding. <br />Iii) Final Expenditure Plan: During the final fiscal year of reporting, grantees may <br />be required to include a plan to fully expend HHAP-4 grant funding. This plan <br />must be submitted with the quarterly expenditure report in a format to be <br />provided by Cal ICH. <br />TE Initial Here_ <br />EXHIBIT 1
