Supportive services required for special needs <br />populations include case management, medical or psychological counseling and <br />supervision, childcare, transportation, and job training for the purpose of facilitating a <br />person’s stability and independence. <br />In housing, accessibility modifications to accommodate persons with mobility disabilities <br />may include, but are not limited to, wider doorways, no step thresholds, installation of <br />ramps, grab bars, lowered countertops, and accessible hardware. The needs of residents <br />with sensory disabilities are different from those with mobility disabilities. Individuals with <br />hearing disabilities require visual adaptations for such items as the telephone ringer, the <br />doorbell and smoke alarms. Residents who are blind may require tactile marking of <br />changes in floor level and stair edges and braille markings on appliances and controls. <br />People with low vision may require large print markings and displays, contrasting colors <br />to distinguish changes in level or transition from one area to another, proper lighting, and <br />reduced glare from lighting and windows. <br />Describe programs for ensuring that persons returning from mental and physical <br />health institutions receive appropriate supportive housing <br />A lack of supportive housing combined with inadequate discharge planning and <br />coordination contributes to homelessness when individuals are released from public <br />institutions or public systems of care without having access to appropriate housing <br />options. Coordinated discharge planning is considered a best practice for those exiting <br />foster care, healthcare, mental health treatment, and correctional facilities. The state <br />mandates discharge planning policy for these systems to help ensure a smoother transition <br />into stable housing. <br />For entitlement/consortia grantees: Specify the activities that the jurisdiction <br />plans to undertake during the next year to address the housing and supportive <br />services needs identified in accordance with 91.215(e) with respect to persons <br />who are not homeless but have other special needs.
