CITY OF SANTA ANA ATTACHMENTS ASSURANCES & CERTIFICATIONS Sel ected provide rs wi ll be required to sign and submit "actua l" assurances and certificates as required by the City of Santa Ana and the Workforce Development Board on all contracts . I recognize that I must give assurances for each item bel ow. If I cannot, this proposal will be automatically re j ected . Please initia l each box. I am authorized by my Boa rd of Directors, Trustees, other legally qualified officer, or as t l1e owner of this organization or business to submit this proposal. We are not curren tl y on any Federa l, State of California , or l oca l Debarment list. We wil l provide records to show that we are fiscally so l ve nt, if needed . We have, or wi ll have , all of the fisca l control and accounting procedures needed to ensure that W IOA funds wi ll be used as required by law and contract. We have additiona l funding sources and wil l not be dependent on W IOA funds al one. We do or will meet the app l icable Federa l, State, and loca l comp li ance requirements.
